Add as many questions as you like: text, a date, a phone number, a dropdown, tick boxes. Some questions can be marked staff-only, so you can note things on a form the person filling it in never sees.

Every form has a link that works for anyone, whether or not they have an account. Print the QR code and put it by the door, or send the link in your usual newsletter. You can also fill a form in yourself while you are on the phone.

Questions

What do people use these for?
Anything you would otherwise print: a waiting list, a trip permission slip, a sign-in sheet for an open day, a summer camp booking, a Secret Santa list. It is a blank form, so it is whatever you need this week.
Do people need an account to fill one in?
No. A form link opens for anyone, and a parent who has never heard of us can fill it in.
Can I ask something only staff should see?
Yes. Mark a question staff-only and it never appears on the version you send out, but you can still fill it in and filter on it.
